Paragraph one: credential, setting and population

State the role, your licence and its status, and the setting and patient population you work in. A clinical manager is deciding whether you can be oriented onto their unit rather than trained from scratch, and that judgement is made in the first two lines.

Paragraph two: one clinical result you can stand behind

Choose an outcome you can evidence — a patient load carried, a documentation or compliance improvement, a quality measure your unit moved — and be careful to attribute team results as team results. Overstating a clinical outcome is the fastest way to lose credibility with a manager who knows how the work runs.

What this field is judging behind the words

Services recruit for a specific clinic, and specialty match is close to decisive because the knowledge base and the conversations differ so much between prenatal, cancer and cardiac work. Beyond that, employers look at weekly caseload, comfort with test selection and variant interpretation, and how you handle insurance authorisation, which consumes real clinical time.

Paragraph three: why this employer

This is where most letters fail. "I admire your commitment to excellence" could be sent to anyone. Name something real — a product, a recent announcement, how the team works, a service line or a quality programme the organisation is known for — and connect it to your own experience. If you genuinely cannot find anything specific to say, that is worth noticing before you apply.

Mistakes that cost genetic counsellor candidates

• Describing counselling generically without the specialty, when specialty is the first filter applied
• Skipping the authorisation and test selection work that occupies a large share of the week
• Omitting caseload volumes, leaving a service lead unable to judge how you would fit their clinic template

Those are resume mistakes, but they apply to the letter for the same reason: both documents are read by someone deciding quickly whether you understand the job.

What this role needs on paper

Entry requires a master's degree from an ACGC-accredited genetic counselling programme, followed by certification through the American Board of Genetic Counseling. A growing number of states license genetic counsellors and the requirements and timelines differ by state, so check where you intend to practise. Certification maintenance requires ongoing continuing education.

Where this leads if you get it

Senior counsellor, then programme lead, clinical supervisor or service director. Laboratory, industry and variant curation roles are a well-established alternative track, as are research and teaching positions.

Check the licence number and expiry date on both documents before sending, because a lapsed credential ends an application regardless of the letter.
